Introduction

This Consent to Telehealth Services Agreement (“Consent”) explains your understanding and agreement regarding the use of telehealth for your healthcare with Hermister (“we,” “our,” “us”).
Telehealth involves the remote delivery of healthcare services and information using telecommunications technology, such as video conferencing, phone calls, and secure messaging. By consenting, you agree to engage in these remote healthcare interactions.

Scope of Telehealth Services

Hermister provides telehealth services to allow you to consult with licensed healthcare providers, including doctors, nurse practitioners, and other clinicians, without an in-person visit. These services may include medical history review, evaluation, diagnosis, treatment planning, prescriptions, and referrals.

Your Rights and Responsibilities

  • You have the right to withhold or withdraw consent for telehealth services at any time without affecting your right to future care.
  • You acknowledge that telehealth visits are voluntary and that you may request an in-person consultation if preferred or necessary.
  • You agree to provide accurate and complete medical information and notify the provider of any technology issues during telehealth sessions.
  • You understand that telehealth may have limitations, including potential technical failures, and that not all conditions can be diagnosed or treated remotely.

Technology and Privacy

  • Telehealth visits will be conducted using secure, HIPAA-compliant technology when available.
  • Data transmitted during telehealth sessions will be protected but cannot be guaranteed free from potential interception or breach.
  • You agree to use a private location during telehealth encounters to protect your confidentiality.
  • You understand that electronic communications, such as email or text messaging, may not be secure, and you consent to their use for communication with Hermister only if you accept those risks.

Risks and Benefits

  • Benefits include convenience, access to care when in-person visits are difficult, and timely medical advice.
  • Risks include technology malfunction, incomplete assessment due to lack of physical examination, and potential privacy limitations.

Consent to Treatment and Prescriptions

  • By consenting to telehealth, you authorize our healthcare providers to evaluate your condition, provide treatment recommendations, and, if clinically appropriate, prescribe medications electronically.
  • You understand prescriptions may be sent to a pharmacy you designate, which may have its own privacy policies.

Limitations and Emergencies

  • Telehealth is not appropriate for all medical conditions, especially urgent, emergent, or life-threatening situations.
  • If you have a medical emergency, you agree to call emergency services or visit the nearest emergency room.
Acknowledgement and ConsentBy signing below or electronically agreeing, you acknowledge that you have read, understood, and agree to the terms of this Consent to Telehealth Services. You agree to participate in telehealth under these conditions and authorize Hermister to provide telehealth services as described.
